Add an observation

What did you do? What did you notice? What else was changing?

That is all an observation needs to be. Two minutes. A few sentences. One moment in your practice, recorded.

“Fresh urine orally, once daily, about six weeks. Skin irritation gradually reduced. Also stopped alcohol and changed diet during the same period.” — positive

“Tried drinking it three mornings. Nauseated each time. Stopped.” — stopped / adverse

“Daily practice for 18 years. Cannot isolate any single effect. Continue because it feels supportive.” — neutral

All three belong in the collective record. Positive, negative, neutral, mixed, discontinued — every observation matters.

You can come back and add more over time. This is a field notebook, not a one-time survey.
